Reality Warper - "Bend reality to your whims." - gives each player kill a 5% chance to spawn a purple portal to the Valley of Screams. That is the same portal that otherwise appears randomly around the world and admits only one player before vanishing, making this charm the only documented way to manufacture the opportunity on demand through combat.

The Valley matters because of what is inside: it is where Spirit Charms are removed, and two random charms sit on pedestals free for the taking. A PvP-active player wearing Reality Warper is effectively rolling for charm-acquisition trips with every kill - a slow but self-sustaining engine for growing a charm collection. Documentation also notes Dr. Voris will teleport players there for $2,000 as an alternative route.

Acquisition is simple and cheap: purchased from Dr. Voris for 212 Soul Tokens. It requires Level 10, weighs 1 wt., and is documented as found in the Soul Society Outskirts. For players who rarely kill other players, the trigger rarely rolls - this is a PvP-economy charm through and through.
